2026 Player Paperwork Instructions

Please review the requirements below carefully to ensure your athlete is cleared to participate:

Important Printing & Signature Requirements:

• All paperwork MUST be printed single-sided (double-sided copies will not be accepted)
• All forms MUST be signed by both the parent/guardian AND the athlete where applicable.

Required Documents:

1. AYF Player Paperwork Packet: This was sent in your TeamLinkt registration confirmation email (please check spam/junk folders). If needed, you can also download it directly from your TeamLinkt account. - See instructions below. 
2. Birth Certificate (NEW athletes only): Must provide ORIGINAL and a COPY - KPYFC will review the original and keep the copy.
   
3. Medical Form (REQUIRED): A 2026 dated physical OR a Medical Clearance Form (found in the AYF Player Packet). Must be signed by a pediatrician and dated 2026 . This is mandatory — your child cannot participate without this!!
4. 2025–2026 End-of-Year Report Card

Additional Requirement for Cheerleaders:

• A copy of the 2026 KP Cheer Contract - Must be signed by both the parent/guardian and the cheerleader.

How to Download & Print Your AYF Player Packet (TeamLinkt):

1. Go to: https://app.teamlinkt.com/
2. Log in using the username and password you created during registration
3. Locate your Family Circle:
   • On mobile/app: Top right corner
   • On desktop: Bottom left corner
4. Click on My Family
5. Select your athlete’s name
6. Click Registration 
7. Click View Invoice
8. Select DOWNLOAD AYF PLAYER PACKAGE
9. Once downloaded, open the file and print your player packet single sided only!!
   • Any paperwork printed double sided will not be accepted!!
